Daly, H. E., and Cobb, J. B. (1989). For the Common Good: Redirecting the Economy toward Community, the Environment, and a Sustainable Future. Boston: Beacon Press.

The book argues that standard economics abstracts the individual from community and the economy from the biosphere, and proposes policy to reconnect them. Its appendix builds an Index of Sustainable Economic Welfare for the United States. The index starts from personal consumption, adjusts for distribution, adds unpaid household work, and subtracts defensive expenditures, environmental damage and the depletion of natural capital. It is the forerunner of the Genuine Progress Indicator. The wiki cites the book for the method of subtracting what the official accounts count as output. See also Herman Daly.
